Introduction

Fistula-in-ano is a chronic inflammatory condition characterized by an abnormal tract connecting the anal canal to the perianal skin. It commonly develops following anorectal abscesses and is associated with persistent discharge, pain, and recurrence.1 Conventional surgical procedures such as fistulotomy and fistulectomy are effective but may be associated with recurrence, delayed wound healing, and risk of anal sphincter injury. Ksharasutra therapy, a well-established Ayurvedic para-surgical procedure described in the classical texts of Ayurveda, has emerged as an effective minimally invasive treatment for fistula-in-ano. Among the various alkaline preparations used in Ksharasutra, Apamarga Kshara, derived from Achyranthes aspera Linn., is considered the most effective due to its potent debridement, antimicrobial, and wound-healing properties.
