Shadbindu Taila and Chitraka Haritaki Avaleha in the Management of Chronic Rhinosinusitis

Chronic rhinosinusitis (CRS) is a persistent inflammatory condition affecting the nasal cavity and paranasal sinuses, typically lasting at least 12 weeks. Common symptoms include nasal obstruction, nasal discharge, facial pressure or pain, and reduced sense of smell. The condition can affect sleep, daily activities, and overall quality of life. Conventional management may include saline nasal irrigation, intranasal corticosteroids, and other treatments selected according to disease severity and individual clinical findings.

In Ayurveda, chronic rhinosinusitis is commonly correlated with Pinasa, a condition associated with persistent nasal symptoms. Shadbindu Taila, administered through the traditional nasal procedure known as Nasya, and Chitraka Haritaki Avaleha, taken orally, have been investigated as a combined Ayurvedic approach to symptom management. Early research suggests potential benefits, although the strength of the evidence and the long-term effectiveness of this combination require further evaluation.1

Shadbindu taila: Composition and potential role

Shadbindu Taila is a classical Ayurvedic medicated oil used in Nasya, a procedure involving the administration of preparations through the nostrils. The formulation traditionally contains sesame oil processed with herbs such as Bhringaraja (Eclipta alba), Yashtimadhu (Glycyrrhiza glabra), Shunthi (Zingiber officinale), Vidanga (Embelia ribes), and other botanical ingredients.2

Its traditional role in nasal disorders includes supporting nasal comfort and addressing symptoms associated with Pinasa. The oil-based formulation may help lubricate the nasal passages, while the individual botanical ingredients have been investigated for various biological properties. However, the precise mechanisms through which Shadbindu Taila may influence chronic sinus inflammation have not been conclusively established.

Chitraka haritaki avaleha: Composition and potential role

Chitraka Haritaki Avaleha is a semisolid Ayurvedic herbal preparation administered orally. Its ingredients include Chitraka (Plumbago zeylanica), Haritaki (Terminalia chebula), Amalaki (Phyllanthus emblica), Guduchi (Tinospora cordifolia), and Dashamoola, a traditional combination of ten medicinal roots and plants. The exact composition may vary according to the formulation used.2

In traditional Ayurvedic practice, the formulation is used for respiratory and nasal conditions. Its constituent herbs contain diverse phytochemicals that have been studied for potential antioxidant and anti-inflammatory properties. These properties provide a possible rationale for investigating the formulation in inflammatory conditions such as chronic rhinosinusitis, although the contribution of individual ingredients to clinical outcomes remains uncertain.

Combined approach in chronic rhinosinusitis

The combination of Shadbindu Taila Nasya and oral Chitraka Haritaki Avaleha has been evaluated as an integrated approach to managing chronic rhinosinusitis. The two preparations are intended to complement each other through different routes of administration.

Potential areas of benefit under investigation include:

  • Nasal obstruction and discharge: Improvement in these symptoms may help reduce the overall burden of nasal disease
  • Facial pressure and discomfort: Symptom improvement could contribute to better daily functioning
  • Sense of smell: Changes in olfactory symptoms are an important outcome when assessing chronic rhinosinusitis
  • Quality of life: The Sino-Nasal Outcome Test (SNOT) and Rhinosinusitis Disability Index (RSDI) are validated tools used to assess symptom severity and the impact of disease on daily life

Safety considerations

Both formulations should be used under the supervision of a qualified healthcare professional. Herbal preparations can cause adverse effects, and their composition and quality may vary. Nasal oil administration also requires appropriate clinical assessment and technique because aspiration of oil can cause pulmonary complications. Chitraka-containing oral preparations require particular caution because Plumbago zeylanica contains biologically active compounds that may cause toxicity.

These preparations should not replace prescribed treatment. Persistent symptoms, recurrent episodes, or worsening nasal obstruction require medical evaluation. Saline irrigation and intranasal corticosteroids remain established options in appropriate patients, depending on clinical assessment.

Conclusion

Shadbindu Taila Nasya combined with Chitraka Haritaki Avaleha represents an Ayurvedic approach being investigated for chronic rhinosinusitis. Preliminary clinical findings suggest possible improvements in nasal symptoms and quality of life, but limitations in the available evidence mean that its effectiveness and safety are not yet definitively established. Well-designed controlled studies are needed to clarify its clinical role, optimal treatment protocols, and long-term safety. The combination should be considered only with appropriate professional guidance and alongside evidence-based clinical care.3
